Your guide to student registration with FinalForms.
Parent Registration
How do I sign up?
- Go to: Final Forms Link
- Locate the parent icon and click NEW ACCOUNT below.

Type in your NAME, DATE OF BIRTH, and EMAIL. Next, click REGISTER.
NOTE: You will receive an email within 2 minutes. If you do not see it, please check your spam folder. If you still can't find it, email support@finalforms.com.
- Check your email for an ACCOUNT CONFIRMATION EMAIL from the FinalForms Mailman. Once received and opened, click CONFIRM YOUR ACCOUNT.
- Create your new FinalForms password. Next, click CONFIRM ACCOUNT.
- Click REGISTER STUDENT for your first child.
Registering a Student
What information will I need?
- Basic medical history and health information
- Insurance company and policy number
- Doctor, dentist, and medical specialist contact info
- Hospital preference and contact information
Registering Your First Student
How do I register?
IMPORTANT: If you have already followed the parent registration steps, you can start at Step 3 below.
- Go to: Final Forms Link
- Click LOGIN under the Parent Icon.
- Locate and click the ADD STUDENT button.
- Type in the LEGAL NAME and other required information. Then, click CREATE STUDENT.
- If your student plans to participate in a sport, activity, or club, click the checkbox for each. Then, click UPDATE.
- Complete each form and sign your full name (e.g., ‘Jonathan Smith’) in the parent signature field. After signing, click SUBMIT FORM.

When all forms are complete, you will see a ‘Forms Finished’ message.
IMPORTANT: If required, an email will be sent to your student prompting them to sign required forms.
Additional Students
How do I add more students?
Click MY STUDENTS on your FinalForms dashboard. Then, repeat steps 3 through 7 from the 'Registering Your First Student' section for each additional student.
Updating Information
How do I make changes?
Log in at any time and click the UPDATE FORMS button to update information for any student.
